The plastic prongs holding one of the four wheels broke off allowing the upper rack to sag when pulled out.
I ordered part PS10064063, metal left and right rack adjusters that should be more durable than the plastic parts that broke. To install, I watched this YouTube video (https://www.youtube.com/watch?v=otUn8eoceBw), pausing and rewinding as needed. Piece of cake! FYI, when the axle broke, the small little plastic prongs stopped the grinder, which caused the dishwasher not to cycle. Fortunately, that was repaired under extended warranty, but the rack wheel was not covered. The repairman said the grinder (in most dishwashers these days) is a very delicate blade. They can't really grind much - even tiny plastic pieces.

The wheels broke and came off of the top rack had to replace the top rack adjusting unit
Laid all the parts out after removing the old adjusting units off of the top rack. I figured out how the new parts went together and attached them to the rack and replaced the rack in the dishwasher. The instructions that came with the new adjuster kit were not much help they were hard to understand and the pictures weren’t very good easier to figure it out on my own. Hope this helps

The photos weren't very clear.
The new part has been modified so I was not replacing with "like" parts and with the photos being not very clear, it took a bit longer to figure out what went where. Also it took a little time to figure out how the old adjuster came apart but it really didn't matter if I broke it, it was broken already.

The plastic tabs had broken on 2 of the 4 wheels so they would fall off & the top rack would fall down.
To remove the old parts off the top rack I used a jack knife & a needle nosed plier. The pictures were small in the instructions especially where it showed the small backing plate that held the part that slides up & down. I installed it wrong but I could easily remove it once I pushed in the the tab with a knife blade & then installed it correctly. The updated metal wheels are a big improvement & it works fine now. Huge savings because I did it myself, anyone can do it if you're not in a hurry.

upper rack roller mechanism failed (wheels broke off)
Replaced adjustor mechanisms on both sides. Instructions enclosed with the parts kit were pretty much useless - photographs were dark and blurry and not very helpful. Removing the old parts was the most difficult part of the job, and all that the instructions said about that was "remove old parts". I found an excellent video on YouTube by a gentlemen named "Matt" which saved the day; couldn't have done the job without the video.

the roller wheel on the upper rack kept falling off, allowing the rack to fall..dirty dishes
tried to follow the instruction that came with the parts, to no avail? it would have been better to send a note that said good luck. the only part that helped was the part that said remove the old parts. after that is was just a fit and test, fit and test, fit and test, only because of my expert analytical abilities was i able to fit it all together. please draw up better instructions. it fits good and works well.

Plastic mounts on rack adjusters broke
On my kitchen counter! Repair was relatively easy. New adjuster appears to be substantially stronger than old!

Broken wheel

One wheel broke on left side rack adjuster so the rack kept falling out of the guide rail
Found replacement instructions on the Internet and followed them to replace the left side rack adjuster. This was very easy but took the longest since I had never done it before. Right side literally took a fraction of the time since I had already done it once!

The shaft to the roller wheels to the upper basket had failed causing a wheel to fall off
. This is the second time we have had this defect with this machine. Apparently due to the frequent failures KitchenAid have redesigned the whole adjuster assembly using a mix of materials rather than all plastic. I was therefore supplied two complete assemblies, for which both existing assemblies had first to be removed. The installation instructions supplied with these replacements were not clear, although the text was intelligible. The photographs of the required work as "figures" were however almost unintelligible. The first side took a long time to complete with much trial and error. Once complete this followed as a model for the second side that went much quicker. Most important to watch for the side of the sliders to be outwards since the illustration is wrong. A similar situation exists with the adjustment button and assembly which can be installed in any of four ways, although only one will work. Finally an "errata" insert indicated that a special tool was included in the hardware set for installing the two screws using a Phillips screwdriver; this was not included in my set, although fortunately I had a socket set with this tool included.

roller wheels on rack broke-- needed replacement / repair
"level of difficulty" and "time to repair" are very tricky. IF I'd have relied on the supposed detailed photos sent with the parts, the "level of difficulty" would have been "impossible" and the "time to repair" would have been "takes forever; still trying to figure it out." However, as another commentator mentioned, there is a Youtube clip from KitchenAid that walks you through it. With that-- "difficulty" became "very easy" and time to repair became "10 minutes". Definitely watch the Youtube clip!!

Poor Directions
Use of black/white pictures made it hard to see part details. Use grayscale insted. Also provide a schematic of the parts involved. The similarity of names made it hard to determine which part was being referred to. used magnifying glass and a lot of trial and error to resolve instalation problems. Second set of rollers went smoother.

Upper rack wheel supports broke
Fairly straightforward install of the kit parts. I was glad to see the new parts were stainless steel and not the weak plastic. I wish I would have known this - I ordered two kits instead of one because I figured it would break again soon. I was very glad to finally find the included screwdriver to torx adapter - I forgot my torx set.
