The upper agitator is floating in my washing machine and will not stay connected. What part should I replace. It will not stay in place.

I don't know the proper names to parts so I apologize. The hose, clamp and mechanism that puts water in the washer is leaving from the connection. What parts do I need to order to replace, to stop the leak?
